冷凝集对血常规检测结果的影响及处理方案探讨
Effect of Cold Agglutination to the Results of Routine Blood Test and Its Related Schemes Dealing with These Interferences

Abstract
Objective To find a convenient and ideal method of rectifying wrong results in cold agglutinated specimens by e-vaiuating different schemes. Methods A total of 15 cold agglutinated specimens were collected and processed with water bath at 37°C for 10 minutes, 30 minutes, 30 minutes followed by 1 minute at low temperature, or with plasma exchange method for 1 or 3 times, respectively. After being treated, these samples were immediately measured with SYSMKX-2100. The results of corresponding fresh samples which were measured immediately after they were drawn again were used as controls ("true value"). Results Alarming signals related to RBC agglutination were emitted by SYSMEX-2100 during the a-nalysis of cold agglutinated samples. The abnormal results included: the significantly decreased value of RBC and HCT, increased value of MCH and MCHC and the incorrect ratio of HGB/RBC. When the fresh samples were measured immediately after they were drawn again,alarming signals disappeared and all results of the detecting items returned to normal. In addition, no agglutinated RBC was observed in smear. The results were close to "true value" in samples processed by water bath at 37°C for 30 minutes (P>0. 05). The results of WBC,RBC,HGB and MCHC could be corrected by plasma exchange for 3 times (P>0. 05) ,but this method could lead to significantly reduced counts of PLT ( P<0. 05). Other schemes could not totally correct the counts of RBC in part of specimens. Furthermore,agglutinated RBC could still be observed under the microscope if samples were not detected immediately after water bath at 37°C for 30 minutes. Conclusion The best method of dealing with cold agglutinated is to detect the specimen immediately after the blood is drawn again. Secondly,the most effective way of correcting the results of agglutinated specimens is to incubate them at 37°C for 30 minutes before being measured without any delay. An immediately blood smear is also suggested so as to exclude the existence of RBC agglutination completely. An alternative way is to exchange plasma for 3 times if previous method could not totally correct the wrong results m cold agglutinated specimens. However,low counts of PLT could be a defect of this method, and in this condition, counts of PLT should be measured after water bath.关键词
