Abstract
Objective To study the indications, efficacy and safety of transcatheter interventional therapy in ethnic minority patients with congenital heart disease living on the frontier. Methods 191 patients were recruited in 45 days. Each patient underwent examinations and final diagnosis within 2 days. The findings in each patient were discussed by departments of cardiology, cardiac surgery, ultrasound and anaesthesiology before operation. 114 patients were selected for intervention therapy. After preoperative preparation . (he interventional process was implemented step by step in one day. Results 110 patients were treated by inlerventional occlusion within one week. The success rate was 96.5% and even reached 100% in 22 patients wiih atrial septal defect (ASD) and 6 patients with pulmonary valve stenosis ( PS). The success rate was 97% in 32 patients with patent ductus arteriosus ( PDA) and 93% in 54 patients with ventricualr septal defect ( VSD). Two patients with two abnormalities were treated successfully. 38 patients had aneurysm of perimembranous VSD. Assisted by PTCA guidewire, the operation was accomplished in those patients whose track was difficult to establish. The effect on the tricuspid valve was reduced effectively by using A4B2 occluder and there was no effect on the right ventricular outflow tract. Conclusions Interventiona! Therapy for patients with congenital heart disease should start with easy cases to ensure the safety of the operation. The success rate of ASD, PDA, and PS is high. PTCA guidewire assists in the success of the interventional occlusion of VSD with aneurysm. Ultrasonic monitoring during the operation is very important.关键词
