Abstract
Objective: To analyze the advantages and disadvantages of non-pneumoperitoneum transumbilical laparoscopic gynecological surgery and conventional one. Methods: Compared the 30 cases that underwent the non-pneumoperitoneum transumbilical lap-aroscopie surgery and 30 cases that did conventional one from Jan. 2010 to Jan. 2011. Observe the age,body mass index(BMl) ,surgery duration,blood loss,rate of conversion to laparotomy,postoperative aerofluxus time,the incidence of abdominal distension and shoulder-back pain,length of stay and wound complications etc of the two groups. Results:The surgeries of the two groups were successful, while there was one case that turned to conventional laparoscopic surgery because of difficulties of hemostasis. The non-pneumoperitoneum transumbilical laparoscopie surgery took significantly longer time than that of conventional laparoscopic surgery. The postoperative aerofluxus time and length of stay were shorter than that of conventional ones. There was no significant differences between the two groups on blood loss. The abdominal distension and shoulder-back pain didn't occur in the non-pneumoperitoneum transumbilical laparoscopic surgery. These 60 patients were followed up for 3 months after operation and no incision hernia,wound infection and other complications occurred. Conclusions: The technology of non-pneumoperitoneum transumbilical laparoscopic surgery which takes longer time is still in its infancy and immature,while the non-pneumoperitoneum transumbilical laparoscopic surgery brings smaller body trauma and its safe and feasible. Compared with conventional laparoscopic surgery.it shows better cosmetic effect,less pain,rapid recovery.no abdominal-back pain and no accumulation of CO2.关键词
