不同水平低中心静脉压对猪血流动力学及肝静脉损伤出血量影响的研究
EFFECT OF DIFFERENT LEVEL OF LOW CENTRAL VENOUS PRESSURE ON HEMODYNAMICS AND BLOOD LOSS IN PIG ANIMAL MODEL OF HEPATIC VEIN INJURY

Abstract
Objective:To observe the effect of different level of low central venous pressure(LCVP) on he-modynamics and blood loss in pig animal model of hepatic vein injury,and evaluate the best security value of LCVP. Methods: Thirty Bama miniature pigs were randomly divided into 5 groups with 6 pigs each group. After anesthesia, openning the abdominal cavity, partial liver of the left lobe was resected, and isolated the left hepatic vein. A Swan-Ganz catheter was placed through the right femoral vein for the measurement of PCWP, SPAP, DPAP and MPAP. After the model established, cardiac output (CO) was measured, and the HR, mean arterial pressure(MAP) , mean pulmonary arterial pressure(MPAP), pulmonary capillary wedge pressure(PCWP) and so on were recorded, and the cardiac index(CI), systemic vascular resistance (SVR) and pulmonary vascular resistance (PVR) were calculated. Different doses of nitroglycerin were used to control the CVP at the following different levels. After the CVP was stable for 10 min, hemody-namic indexes were recorded and calculated again, and the hepatic venous pressure was measured through the outer sheath catheter. And then the outer sheath catheter was put in Axillary midline and openned for 30 s, and the blood loss from the left hepatic vein was collected. Results: The successful ratio of animal model of different LCVP was 100%. Controlled low central venous pressure has the varying degree influence to hemodynamics and blood loss. Low central venous pressure can be effective in reducing blood loss in hepatic vein injury of pigs. Conclusion: As maintaining a relatively stable hemodynamic circumstances, controlled of LCVP at 0. 267~0. 40 kPa can maximum reduce the blood loss in hepatic vein injury of Bama miniature pigs.关键词
