Abstract
Objective To study the inflammatory status of ketosis tendency in newly diagnosed type 2 diabetes and the effect of intensive insulin therapy on inflammatory cytokines, to explore the anti-inflammatory effects of insulin and the mechanism. Methods Selected 126 cases of the ketosis tendency with newly diagnosed type 2 diabetes in the hospital, and all patients were randomly divided into two groups: A group of 64 patients for the regular insulin treatment group ( regular insulin R 30 minutes before meals, regular insulin N at bedtime ), B group of 62 cases for insulin pump continuous subcutaneous infusion. The early growth response factor serum-1 ( Egr-1 ),tissue factor ( TF ) and plasminogen activator inhibitor-l( PAI-1 )con-centrations were detected before and after treatment. Selected 120 healthy volunteers with baseline data match as a control group( C group ). Results ( 1 ) Compared with the C group, the type 2 DM patients with inflammatory cytokines Egr-1 ,TF, PAI-1 was significantly increased in A、B groups( P <0. 01 ). ( 2 ) Compared with before treatment, intensive insulin therapy on inflammatory factor Egr-1, TF, PAI-1 significantly reduced( P < 0. 05 )in A group and B group. And the B group decreased more significantly compared with the A group( P < 0.05 ). Conclusion Ketosis tendency in newly diagnosed type 2 diabetic patients with chronic inflammatory state, the intensive insulin therapy could significantly reduce the levels of inflammatory factors in patients with type 2 DM, and the different program due to the difference anti-inflammatory effect. The insulin had anti-inflammatory effects, with the mechanism of inhibiting the activity of Egr-1, reducing TF and PAI-1 expression.关键词
