Abstract
Objective To investigate the current situation of health management and health service access of floating pregnant women, to provide reasonable proposals on how to improve public health management functions, to further develop community service skills, to create a healthy supportive environment, to provide fair, safe, effective, and long - lasted management and services, and to enhance the awareness levels of maternal health. Methods The current health situation and service utilization between the city residents and floating population in Xinjing Town, Changning District, Shanghai were analyzed and compared through social equity analysis and objective evaluation. The operating difficulties of the existing management system were analyzed and reasonable suggestions on how to improve public health management functions were put forward. Results The average rate of the early pregnancy ( pregnancy within 12 weeks ) registration among floating pregnant women was 22. 5% , far lower than that of the city residents ( 92. 7% ) ( P <0. 001 ). The average rate of postpartum visit was 93. 5% among floating pregnant women, lower than that of the city residents ( 99. 8% ) ( P <0. 01 ). The average prenatal examination times was 4. 4, with 22. 0% of the pregnant women didn't seek prenatal examination, of which 53. 5% did not know the significance, 38. 1% considered the fees too expensive, 8. 4% was due traffic inconvenience; the average puerperal fee was 1 085. 31yuan, the average delivery costs was 2 524. 49yuan, the maximum was 7 000. 00yuan. The average expected puerperal fee during pregnancy was 459.00yuan. The average expected delivery cost was 1 563. 00yuan. Conclusion Further improvement of public health management should be carried out with the hope to formulate a public service - oriented government, community service networks should be established with increased government investment, social equity and justice should be stressed when provide public services with the hope to improve the health status of floating pregnant women.关键词
