儿童石骨症骨髓移植前后的视功能观察

Abstract
Purpose: To detect long-term change of visual function of a child with malignant osteopetrosis after hematopoietic stem cell transplantation. Methods 1 year old girl diagnosed as osteopetrosis underwent CD +34 selected haematopoietic stem cell from an unrelated donor. Visual acuity, fundus examination, F-VEP and CT of optic canal were applied to assess the visual function. Results Bone marrow transplantation was successful. Peripheral blood test, splenohepatomegalia and osteosclerosis improved gradually. Corrected visual acuity raised. But myopia refractive diopter increased annually. Optic nerve atrophy did not change. F-VEP showed approximately ordinary wave. Diameter of optic canal at the year of + 5 post-transplant became much narrower than the year of + 1 post-transplant. She also had glaucoma manifestation in follow up period. Conclusions Hematopoietic stem cell transplantation is an effective way to deal with malignant osteopetrosis. F-VEP and fundus examination can estimate visual function indirectly. But they can not reflect the visual acuity directly. The factors that can lead to narrowing of optic canal after marrow transplantation need much research. Whether osteopetrosis can induce the happening of glaucoma and severe myopia also need clinical evidence.关键词
