临床药师对1例高血压并糖尿病患者的药学服务
Pharmaceutical Care for a Hypertensive Patient with Diabetes by Clinical Pharmacists

Abstract
OBJECTIVE: To discuss the approaches for clinical pharmacists to carry out pharmaceutical care. METHODS: Through participating in the course of treatment for a hypertensive patient with diabetes, combining with the disease characteristics of the patient, medication history, drug interactions and adverse drug reactions (ADRs), clinical pharmacist suggested that doctors should select appropriate drugs for the patient, and the patient should be provided with individualized pharmaceutical care. RESULTS: The pharmacotherapy was effective and rational. By providing pharmaceutical care for the hypertensive patient with diabetes, the treatment-related problems (TRP) in time were identified and resolved, and suggestions on the rational drug use were put forward. CONCLUSION: Clinical pharmacists have their own advantages in providing individualized pharmaceutical care for patients. They play an important role in the rational use of drugs.关键词
