双节段与三、四节段椎弓根螺钉固定胸腰椎单椎体骨折伴脱位的生物力学比较
Biomechanical study of transpedicular fixation for short-segment and three and long-segment instrumentation of the thoracolumbar spinal fracture and dislocation

Abstract
To compare the biomechanical stability of transpedicular fixation with short-segment, three andlong-segment instrumentation for thoracolumbar spinal fracture and dislocation.L1 vertebral body slice-fracture andT12 dislocation model was established on 8 fresh calf thoracolumbar spine specimens. The stability of T10-L4 segments in transpedicular fixation with short-segment, three and long-segment was compared to intact specimens (control group). A cyclic load of 300N was applied to specimens at a frequency of 1.0Hz up to 2000 cycles. Segmental instability tests were performed in control group, 3 fixation and 3 fixation groups, followed by cyclic loading. Range of motion (ROM) in flexion-extension, lateral bendingand axial rotation by three-dimensional laser scanner.ResultsThe ROM in flexion-extension, lateral bendingand axialrotation of short-segment and three and long-segment was smaller than the intact specimens, (P<0.05). There was no significant difference in the flexion-extension and lateral bending ROM between three instrumentation groups! P>0.05), but long-segment constructs had smaller axial rotation ROM than that of short and three-segment constructs (P<0.05). There was no significant difference among three instrumentation groups in fatigue test (P>0.05).Transpedicular fixation can enhance the biomechanical stability with short, three-segment and long-segment instrumentation for thoracolumbar spinal fracture and dislocation. Long-segment instrumentation is superior to other two instrumentation groups in providing axial rotation stability.关键词
