Abstract
Objective To investigate the influence of anesthesia with the combination of propofol, ketamine and fentanyl on stress reaction in patients undergoing lung cancer surgery. Methods Seventy five patients undergoing lung cancer surgery were selected and randomly divided into three groups with 25 patients in each group. Group A received the anesthesia with the combination of propofol, ketamine and fentanyl; group B received the anesthesia with propofol and ketamine; group C received the anesthesia with propofol and fentanyl. During the surgical operation of all the three groups, total intravenous anesthesia was used to maintain thegroups. Results Ten minutes after the skin incision, the mean arterial blood pressure( MAP) of group B increased significantly compared with that before the anesthesia and the MAP of group A( P <0. 05 ). The heart rate( HR )of group B 10 min after the skin incision and at the time of extubation significantly increased compared with that before the anesthesia and that of group A( P < 0. 05 ). There was no significant difference in MAP,HR and SpO2 between group A and C in the process of anesthesia( P >0. 05 ). In group A, Cor, Ang II and β - EP at the time point of T1, T2 and T3 did not significantly increase, compared with those at the time point of TO (P>0.05 ). While in group B and C,Cor, Ang Ⅱ and β - EP at the time point of T1,T2 and T3 increased significantly( P<0.05 ). Plasma concentration of IL - 10 at the time point of T3 in all the three groups increased significantly compared with that at the time point of T0( P <0. 05 ). In group A,the VAS scores during and after the anesthesia decreased significantly( P <0. 05 ), and the RSS scores increased significantly( P <0. 05 ),compared with those in group B and C. However,there was no significant difference in the incidence of nausea and vomiting among the three groups( P > 0. 05 ). Conclusion The anesthesia with the combination of propofol, ketamine and fentanyl is safe and effective, and has certain inhibitive effects on stress reaction of patients undergoing lung cancer surgery.关键词
