放疗同期DDP单药方案与PF联合方案治疗局部晚期鼻咽癌近期疗效的对照研究
Short-term efficacy of concurrent chemoradiotherapy with DDP or PF in locoregionally advanced nasopharyngeal carcinoma

Abstract
Objective To investigate the short-term therapeutic effects and adverse reactions of concurrent chemoradiotherapy with cisplatin (DDP) or DDP plus 5-fluorouracil (5-FU) in the treatment of locally advanced nasopharyngeal carcinoma (NPC). Methods One hundred and twelve patients with locally advanced NPC were randomly assigned to DDP group ( chemoradiotherapy with DDP alone,n=40) and PF group (chemoradiotherapy with DDP plus 5-FU,n =72). Same dose and fractionation of radiotherapy was administered in both groups. The patients of DDP group received weekly intravenous dripping of 25 mg/m2DDP from 1 to 7 week of the radiotherapy; the patients of PF group received intravenous dripping of 25 mg/m2 DDP plus 450 mg/m2 5-FU in 1 and 4 week of the radiotherapy. Results Two patients in PF group did not complete chemotherapy. All patients in the cohort accomplished radiotherapy. No significant differences were found in the remission rates of nasopharyngeal primary focus and metastatic cervical lymph nodes in either 60 Gy radiation doses or 3 week after radiotherapy (P > 0. 05 ) . The oral mucosal reaction was significantly milder in DDP group than in PF group (P<0. 05).There were no significantly differences between two groups in other adverse effects including leukocytopenia, thrombopenia, nausea and vomiting, radiodermatitis, liver function damage and etc. (P > 0. 05). Conclusion Concurrent chemoradiotherapy with low dose DDP alone to treat locally advanced NPC has the similar short-term therapeutic effect,but milder adverse reactions, compared with the treatment of DDP plus 5-FU.关键词
