人工耳蜗术后患儿听阈评估
Assessing postoperative thresholds in children with cochlear implants through both sound field speakers and earphones

Abstract
Objective To assess the efficacy of cochlear implant on the recipient's listening sensitivity, and compare sound field speakers and earphones as transducers in post-operative thresholds assessment. Methods A total of 27 patients with severe to profound sensorineural hearing loss who had received cochlear implantation participated in this assessment. (18 males and 9 females). Age at implantation ranged from 1.7 to 18.6 years, with an average of 7.6±5.4 years. After the opening and programming of the devices, cochlear implant aided thresholds at 0.2S, O.S, 1.0, 2.0 and 4.0 kHz were measured through both sound field speakers and TDH39 earphones. Besides, threshold at 8.0 kHz was measured through TDH39 earphones. Paired t-test was administered to compare the results obtained by the two means. Results The 3FA (I.e. Averaged threshold at 0.5, 1.0 and 2.0 kHz) and 4FA (I.e. Averaged threshold at 0.5, 1.0, 2.0 and 4.0 kHz) obtained through sound field speakers was 39.4±7.7 dB HL and 37.5±7.3 dB HL, respectively, compared to 39.3±6.1 dB HL and 38.4±5.7 dB HL through TDH39 earphones, showing no statistical significance ((P=0.8801 and P=0.2133, respectively). Similarly, no statistically significant difference was found between the two transducers for thresholds at 0.25 kHz (P=0.8756), 0.5 kHz (P= 0.0630), 1.0 kHz (P =0.1980), or 2.0 kHz (P=0.6866). However, aided threshold at 4.0 kHz through earphones was lower than that through sound field speakers (P=0.0062). Conclusions Cochlear implant provides fairly good listening sensitivity to patients with severe to profound sensorineural hearing loss. Earphones can be a good means to measure post-implant thresholds, as reliablye as sound field speakers.关键词
