Abstract

Objective To analyze the feasiblity and outcome of pancreaticoduodenectomy in patients older than eighty. Methods We retrospectively reviewed the data of 16 patients older than eighty who underwent pancreaticoduodenectomy and compared with 145 patients younger than eighty undergoing pancreaticoduodenectomy. Results In the patients older than eighty, seven patients had postoperative complications (43.8%), including abdominal infection in 2 cases, delayed gastric emptying in 2 cases (12.5%), pulmonary infection in 4 cases (25.0%), cardiac insufficiency, wound infection, abdominal hemorrhage and urinary infection in 1 cases for each (6.3%). There were 4 cases (25.0%) with pancreatic fistula. One case belonged to grade A, two cases grade B, and one case grade C. One patient (6.3%) died from MODS caused by pancreatic fistula complicated abdominal infection, abdominal hemorrhage and pulmonary infection. Mortality and mobidity did not differ between the two groups. Conclusions Pancreaticoduodenectomy is feasible and safe for patients older than eighty. Age alone should not be a contraindication to pancreatic resection. If professional surgical team, reasonable preoperative evaluation and proper perioperative management could be provided to elder patients, pancreaticoduodenectomy will lead to lower morbidity and mortality.
