135例骨科围手术期抗菌药物预防性应用调查分析

Abstract
Objective To understand the use situation of antibacterial agents in orthopedic department during perioperative period,so as to provide the basis for formulating the management measures of antibiotic rational use. Methods By the retrospective investigation method, the basic data and the use status of antibacterials in the discharged patients performed orthopedic operation were reviewed. The rationality of prophylactic use of antibacterial agents was reviewed according to the relative national standard and guidelines. Results Total 135 medical records were collected and reviewed. The prophylactic use of antimicrobial agents during perioperative period reached 100.00%. Of all cases,89 cases (65.93%) had definite indicators for prophylactic use. Among 87 cases of type I incision surgery,only 41 cases (47. 13%) were accorded with the medication indicators. Total 6 categories and 14 kinds of antibacterial agents were involved in prophylactic use,most of which were cephalosporins. All agents were administrated by intravenous drip. 105 cases (77.78%) received single antibacterial agent ,30 cases (22.22%) received combination use of two or three antibacterial agents. The rational administration opportunity was only in 20 cases (14. 81%). The duration of antibacterial use was beyond 5 d in absolute majority of cases,with the average medication duration of 8. 1 d and the longest duration of 15 d. Conclusion The perioperative prophylactic of antibacterial a-gents was unreasonalle in the orthopedic department of our hospital,which needs to strengthen the management and take the relative measures for intervention.关键词
