产CTX-M型超广谱β-内酰胺酶肺炎克雷伯菌耐药性分析
Analysis on Resistance to CTX-M-Type Extended-Spectrum β-Lactamase in Klebsiella pneumonia

Abstract
Objective To study the subtype of CTX-M-type extended-spectrumβ-lactamase (ESBLs) encoding gene and research the resistance to Klebsiella pneumonia in Jilin Urban Area, and analyze the epidemiological characteristics of CTX-M-producing isolates. Method PCR was used to detect the genotype of CTX-M type ESBLs in 118 strains of K. pneumoniae and investigate the resistance of 14 positive strains to common antimicrobial agents. The CTX-M type ESBLs-producing KlebsieUa pneumoniae was analyzed with arbitrarily primed polymerase chain reaction (AP-PCR) technique and the fingerprinting profiles were drawn according to the number and length of DNA bands. Results PCR amplification of 118 strains showed that there were 25 strains with CTX-M genotypes, accounting for 21. 19% ; 13 strains with CTX-M-1, accounting for 11.02% ; 16 strains with CTX-M-9 ,accounting for 13.56% ;4 strains with CTX-M-1 group and the CTX-M-9 simultaneously, accounting for 3.39% ;no positive strain with CTX-M-2 group,CTX-M-8 group and the CTX-M-25 group could be found. 25 strains producing CTX-M type extended spectrum J3-1actamases were muhidrug resistant ( resistant to three or more aatibiotics). The servivability ratios of bacteria were in turn as follows : the highest was erythromycin (100%) ,the second was cefotaxime (92%), ampicillin (92%) and tetracycline (88%);the resistant rate of them to imipenem was lowest and all of them were susceptible to meropenem. There were 13 RAPD types in 26 isolates of 25 strains of CTX-M-producing Klebsiella pneumoniae. Conclusion CTX-M type extended spectrum β-lactamases-producing Klebsiella pneumoniae in ]ilin area might be multiple drug-resistant bacteria, and the rates of their resistance to erythromycin, cefotaxime and ampicillin should be high. To some extent, there may be a clonal propagation for CTX-M enzymes.关键词
