脾动脉结扎法在腹腔镜巨脾切除术中的应用(附20例报告)

Abstract
Objective: To evaluate the safety, feasibility and surgical techniques of splenic artery ligation in laparoscopic sple-nectomy for megalosplenia. Methods: Forty cases with megalosplenia and portal hypertension due lo liver cirrhosis were landonily assigned to laparoscopic. group and open group. The perioperative data between 2 groups were comparatively analyzed. Results: There were no differences in preoperative clinical data. Operative time of laparoscopic group was slightly longer than open group. However, blood loss,lime of postoperative feeding,exhaust, postoperative tube removal,postoperative hospital stay were all less in laparoscopic group than those in open group. Conclusions: Laparoscopic splenectomy splenic artery ligaliun is safe, feasible and effective for megalosplenia. Compared with laparotomy,laparoscopic operation has obvious advantages in intraoperative blood loss,time of postoperative foodin-take,exhaust,removal of drainage tube and discharge. The operative time is a little longer because the operative technique should be improved in the early stage.
