Abstract
Objective To explore the risk factors of placenta implanted in scar uterus and the prevention and treatment measures of the postpartum hemorrhage in cesarean section. Methods The clinical data of 12 cases who had twice or more histories of cesarean section and with placenta implanted in scar uterus was analyzed retrospectively. Results All of the 12 cases suffered blood loss more than 500mL, and postpartum hemorrhage volume in 8 cases was more than 1 000mL. Three cases received hysterectomy, and the other 9 cases reserved the uterus after application of carboprost tromethamine, " 8" suture, B-lynch suture and uterine artery ligation. No maternal died and 12 newborns were alive. Conclusion Postpartum hemorrhage caused by placenta implanted in scar uterus is an extremely dangerous complication. Attention should be paid to preoperative diagnosis and surgical preparation. Carboprost tromethamine combined with B-lynch suture can get satisfying hemostatic effect. Placenta should not be stripped blindly in case fatal bleeding occurs. Hysterectomy should be taken decisively when encountering wide area of placenta increta or intraoperative hemorrhage.关键词
