早发冠心病危险因素与冠状动脉病变相关性研究
Study on the Relationship between Risk Factors and Characteristics of Coronary Artery Lesion in Patients with Premature Coronary Artery Disease

Abstract
Objective To investigate the relationship between risk factors and characteristics of coronary artery lesion in patients with premature coronary artery disease (PCAD). Methods One hundred and twenty -four PCAD patients confirmed by coronary angiography were divided into PCAD group (54 cases) and late-onset coronary artery disease group (70 cases, LCAD group). A comparison of characteristics of coronary artery lesion , body mass index (BMI) ,fasting blood glucose(FBG) ,postprandial 2h blood glucose(2hPG) ,haemoglobin Aic(HbA1c) ,total cholesterol (TCH) ,triglycerides(TG) ,high density lipoprotein-cholesterol (HDL-C) , low density lipoprotein-cholesterol ( LDL-C) , lipoprotein a[LP (a) ] , total bilirubin(TBIL) ,bladder inhibin C ( CysC) ,fibrinogen (Fib) , high sensitivity C-eactive protein ( HsCRP) and central aortic pressure ( CAP) was done between the two groups. Logistic regression analysis was applied to assess the association of these risk factors above with PCAD. Results HbA1c,HDL,HsCRP were significantly lower,while LP(a) ,BMI,TG were significantly higher in the PCAD group than those in the LCAD group(P<0.05) ;FBG, 2hPG,LDL-C,TBIL,CysC,Fib,DBP,SBP showed no significant difference between the two groups (P >0. 05). There was significant difference in the arteries of coronary artery lesions between PCAD group and LCAD group ( P < 0. 05 ) , and single-vessel lesion (61. 11%) was the main disease in the PCAD group , while two-vessel lesion (37. 14%) and three-vessel lesion(34. 29%) in the LCAD group. The ratios of limitation, and eccentric lesion in the PCAD group were significantly higher than those in the LCAD group (P < 0. 05 ). However, the ratios of right coronary artery , circumflex artery , concentric and diffuse lesion were significantly lower in the PCAD group than those in the LCAD group(P<0. 05). The coronary artery score showed significant difference between PCAD group and LCAD group (P <0. 05) , which was dominated by the 0 - 19 group(55. 55% ) in the PCAD group. Conclusion HsCRP,BMI,LP (a) , DBP are main risk factors in PCAD patients. PCAD patients are characterized as having mild coronary artery disease , most of which are single -vessel disease , limitation and eccentric lesion.关键词
