Abstract
Objective To compare the clinical effect of treatment in elderly intertrochanteric fractures of the proximal locking compression plate ( LCP ) and the dynamic hip screw ( DHS ). Methods 112 cases of elderly intertrochanteric fracture were treated with LCP ( 60 cases ) and DHS ( 52 cases ). Gradual weight-bearing ambulation was trained after 4～6 weeks. Two groups were compared with operative time, blood loss and postoperative recovery and complications. Results 112 cases were followed up for 6 ～ 18 months. The bone healing time was 3～9 months. In LCP group : 2 cases of mild hip varus were found, and Hams scores were excellent in 48 cases, good in 10, fair in 2, with excellent and good rate of 96. 7%. In DHS group: 2 cases of mild hip varus and 4 cases of type Ⅳ fracture of the greater trochanter smash, fixed loosening appeared, and Hams scores were excellent in 33 cases, good in 9, fair in 6 and poor in 4, and the excellent and good rate was 80. 8%. There was statistically significant difference with excellent rate between two groups( P < 0. 05 ). Conclusions Based on a reasonable choice of the type of fracture fixation, proximal femur LCP and DHS treatment of elderly intertrochanteric fracture Evans I ～ HI type are effective, and Evans type Ⅵ fracture and osteoprotic fracture are more applicable to the proximal femur LCP fixation.关键词
