分化型甲状腺癌患者术后首次行131I清除残余甲状腺组织的疗效及其影响因素
Analysis of the efficacy and influential factors of the first 131I thyroid remnant ablation on differentiated thyroid carcinoma after operation

Abstract
Objective To evaluate the efficacy of the first radioiodine (131 I) ablation of residual thyroid on differentiated thyroid carcinoma (DTC) after operation, and to analyze the influential factors for efficacy. Methods A total of 102 patients after the treatment of DTC were included, and stop to use thyroid hormone, After orally taking 100 ~ 120 mCi 131I ablation on an empty stomach for 3 to 5 days, thyroid hormone replacement therapy was taken, and then checking the rate of thyroid iodine uptake, and 131 I thyroid imaging. Univariate analysis and binary Logistic regression analysis were performed to analyze the effects of gender, age, pathologic type, the level of thyroid-stimulating hormone (TSH), the level of thyrogloubulin (Tg) and rate of thyroid iodine uptake and 131 I ablative dose on efficacy of thyroid remnant ablation. Results The clinical data from 102 patients were collected, among whom 37 cases (36.3% ) achieved successful throid remnant ablation at first dose, but 65 cases did not. The treatment dose of 131I in patients with successful throid remnant ablation was lower than that of failed patients (P =0. 000). Univariate analysis revealed that the first used dose of 131I (P = 0.020). The level of TSH (P =0.000) at the time of 131I therapy were influence factors of 1311 successful ablation rate, and the low level of Tg accompany with the high treatment success rate (P = 0.000) , while the patients'age (P = 0.119) , sex (P = 0.315), histological subtype (P=0.835), pre-treatment 131I uptake ratio of residual thyroid tissue (P =0.375) had no evident influence on the first ablation result. Furthermore, binary Logistic regression analysis showed that the first used dose of 1311 and the levels of TSH and Tg at the time of 131I therapy were influence factors of 131I successful ablation rate (P=0.049, P=0.014, P = 0.001). Conclusions The first used dose of 131I, the level of TSH and Tg at the time of 131I therapy were influence factors of 131I successful ablation rate.关键词
