Abstract
Health management based on community health services ( CHS ) has been a developing trend in medical health fields, however, it is still in an initial stage in China. Health bureau of Zhabei District has explored actively an optimization plan for community health management since the new Shanghai health care reform was implemented, primarily establishing a health management sendee mode taking health management department as the core.
