Early Transtympanic Steroid Injection in Patients with ‘Poor Prognosis’ Idiopathic Sensorineural Sudden Hearing Loss

Abstract
Objective To evaluate the effectiveness of early transtympanic steroid injection via laser-assisted myringot-omy in patients with 'poor prognosis' idiopathie sensorineural sudden hearing loss (ISSHL). Methods Seventy-nine patients, diagnosed as having ISSHL with 'poor prognosis', were randomized into a transtympanic treatment group and a control group. Forty patients received conventional steroid treatment and 39 patients received 4 courses of additional transtympanic injections of methylprednisolone 7 days after conventional steroid treatment. Transtympanic injection was performed through a laser-assisted myringotomy (0.5 mm perforation made in the tympanic membrane) under an operation microscope. Results In the transtympanic treatment group 48.7% (19/39) of the patients showed at least 15 dB improvement in pure tone average compared with 22.5% (9/40) of the patients in the control group on the 8-week follow-up audiogram. In the transtympanic treatment group 46.2% (18/39) of the patients showed an improvement of ≥15% in speech discrimination, compared with 20.0% (8/40)of the patients in the control group 8 weeks after the treatment. The differences between the 2 groups were statistically significantly. Conclusion Early application of transtympanic steroid therapy via laser-assisted myringotomy appears to improve treatment outcomes in patients with ' poor prognosis' ISSHL.关键词
