Abstract
Objective To reexamine the effect of different bum areas on fluid infusion volume at shock stage in severe burn, and then put forward suggestions to improve the traditional formula of fluid resuscitation in the first 24 hours [ total volume of crystal and colloidal fluid (mL) =1.5 × body weight x burned area, resuscitation coefficient is a constant 1.5]. Methods A total of 69 patients who were hospitalized in our hospital from January 2005 to October 2011 were reviewed. All the patients were 18 -70 years old, and the burned areas were larger than 30%. The time after burn injury to the admission was less than 4 hours and all of the patients rode out shock stage. The burned areas , body weight and the volume of crystal and colloid at the first 24 hours of each patient were recorded. We calculated the resuscitation coefficient Y - the volume of crystal and colloid/weight/burned areas, analyzed the relationship between resuscitation coefficient Y and burned areas X through statistical software SPSS 17.0, and made use of linear regression analysis to obtain the regression equation. We got the relationship between resuscitation coefficient Y and bumed areas X by regression equation. Results There was linear relationship between independent variable Y and variable X. The regression equation was Y = 0.0124X + 1.1865. The regression equations and correlation coefficients are statistically significant (P <0.001). Conclusion Early resuscitation coefficient of severe bum patients should not be limited to constant 1.5. The relationship between the volume of fluid replacement during shock stage in severe bums and burned areas should not be limited to a linear relationship, but should be considered as curve relationship as well. Traditional resuscitation formula in the first 24 hours should be modified as; The total volume crystal and colloid fluid (mL) = (0.0124 ×burned area + 1. 1865)× body weight × bumed area.关键词
