Abstract
Objective To compare the clinical therapeutic effects for primary shoulder dislocation in the two groups: Group I , immobilization in external rotation versus internal rotation j Group II , surgical versus conservative treatment. Methods Medline(1966 to Sep,201I) , Embase(June, 1966 to Sep. 2011) , Coehrane Library (Issue 9 2011), CBM(up to Sep, 2011), and CNKI(up to Sep, 2011) were searched with computer for randomized controlled trials and clinical trials on treatment for primary shoulder dislocation, supplemented by hand searching. Meta - analysis was conducted using the Cachrane Collaboration's RevMan 5.1 software. Results Five randomized controlled trials were involved in Group I . The Meta - analysis revealed that immobilization in external rotation has no statistically signicant differences with internal rotation in the intention - to - treat group ( RR = 0. 77, 95%CI 0.58-1.02), the per-protocol group(RR =0.76, 95% CIO. 51 -1.13) and the older than 30 years group( RR =0. 79, 95% CI 0. 29 -2. 11). There were statistically signi: cant differences in the younger than 30 years( RR = 0. 52, 95% CI 0. 36 - 0. 76). Four randomized controlled trials and two non - randomized controlled trials were involved in Group Ⅱ. The Meta - analysis revealed that surgical treatment has no statistically signicant differences with conservative treatment in surgical versus traditional conservative treatment group (RR =0. 22, 95% CI 0. 12 -0. 40), arthro-scopic repair versus arthroscopic lavage group (RR =0. 11, 95% CI 0. 04 -0. 30) and subgroup analysis ( RR = 0. 17 ,95 % CIO. 10 - 0. 29 ).ConclusionForpreveningtherecurrenceofprimary shoulder dislocation, the external rotation has advantage over traditional internal rotation in the younger than 30 years: and the surgical treatment has superiority over conservative treatment.关键词
