局限性Castleman病的CT特征分析及病理对照

Abstract
Objective To explore the CT findings of the localized Castleman disease. Methods The data of 10 patients with localized CD proved histopathologically were analyzed retrospectively. The unenhanced and contrast-enhanced CT images were obtained from all the patients. Two radiologists reviewed and analyzed the CT findings simultaneously. Results The ten patients with localized CD showed twelve masses in total,including single mass in the lung,mediastinum and retroperitoneal space and two-adjacent mass in the neck and axillary fossa. Twelve masses appeared round or elliptical as large as 3 - 12 cm in diameter( mean 5.6 cm). There were hyaline-vascular type in 7 cases and plasma type in 3 cases pathologically. The hyaline-vascular type lesions appeared as a homogeneously hyper-dense mass with central stripe or cleft hypodense area on CT plain scans,but was markedly enhanced simultaneously with abdominal aorta enhancement except on the stripe or cleft area,or with some stripe or spot enhanced vessels around it;CT value increased from 27 - 54 HU to 39 -145 HU excluding 2 relatively large masses with calcification and non-enhanced region. The plasma type lesion appeared as a homogeneously hyperdense mass on plain CT scans with mild or moderate enhancement on enhanced CT scans. Conclusion CT features of localized CD are closely related to the location and pathological type. LCD with hyaline-vascular shows characteristic features on enhanced CT,but plasma cell type shows atypical CT findings.关键词
