Abstract
[ Objective ] To evaluate the clinical efficacy of intravenous dexmedetomidine as an adjunct to local anaesthesia in functional endoscopic sinus surgery (FESS). [Methods] Sixty ASA Ⅰ - Ⅱ patients underwent FESS were randomly divided into two groups: Group C (control group, n=30) and Group D (dexmedetomidine group, n=30). In Group D, Dexmedetomidine was intravenously administered at 0.5 μg/kg per hour after a loading infusion at 1 μg/kg for ten minutes. The patients in Group C were started on 0.9% normal saline infusion at an equivalent rate. Fentanyl was given intravenously when necessary. Changes of heart rate (HR), satiation of oxygen (SPO2), respiratory rate (RR), mean arterial pressure (MAP), rate pressure product(RPP) and the Observer's Assessment of Alertness/Sedation scale (OAA/S) were monitored before injection (TO), after injection at 20 min (T1), 40 rain (T2) and the end of the operation (T3). The operation time and the consumption of fentanyl were observed and recorded. Patient satisfaction and the scores of surgical field quality (SSFQ) after injection at 20 min, 40 min were assessed. [Results] The SSFQ, MAP, HR and OAA/S in group D were significantly decreased compared with that in group C, the consumption of fentanyl in group O was significantly less than that in group C (P <0.05), the satisfaction of patients in group D was higher than that in group C (P <0.05). There was no significantly changes in RR and SPO2 between the two groups.[Conclusions] Dexmedetomidine can display a good definition of surgical field, a stable haemodynamic state and a higher patient satisfaction without respiratory inhibition. It could be regarded as one of the anesthetic protocol of choice in patient undergoing FESS.关键词
