Abstract
OBJECTIVE: To observe the therapeutic efficacy of Shenmai injection combined with non-invasive positive pressure ventilation for acute exacerbation of COPD complicating with pulmonary encephalopathy. METHODS: 70 AECOPD patients with pulmonary encephalopathy collected from our hospital were randomized into treatment group and control group. Treatment group was given rountine treatment, Shenmai injection 40 mL plus 5% Glucose injection 250 mL, qd., non-invasive ventilation. Control group was given routine treatment and additionally received non-invasive ventilation. Therapeutic efficacy, 2 h and 48 h arterial blood gas analysis, C-reactive protein, clinical change, hospital stay and positive pressure ventilation duration were observed and compared between 2 groups before and after treatment. RESULTS: Among 70 cases, 7 cases died after treatment, including control group of 5 cases and treatment group of 2 cases. 48 h arterial blood gas analysis, C-reactive protein level, total effective rate of treatment group were better than those of control group, and non-invasive positive pressure ventilation duration (6.0 ±4.2) d and hospital stay (10.9 ±2.6) d of treatment group were less than those of control group ((8.212.8) d, (14.2 ±2.1) d). There was statistical significance in the difference of above index between 2 groups (P<0.05). There was no serious ADR found in 2 groups. CONCLUSION: Shenmai injection combined with non-invasive positive pressure ventilation is effective in the treatment of acute exacerbation of COPD complicating with pulmonary encephalopathy and can effectively reduce mechanical ventilation and hospital stay.
