桡骨远端骨折钢板与克氏针固定疗效的Meta分析
Treatment of Distal Radial Fractures with Plate or Kirschner Wire:A Meta-Analysis
摘要
Abstract
Objective To evaluate the clinical effectiveness of plate fixation and kirschner wire fixation in distal radial fractures.Methods Computer retrieving MEDLINE (1966-2012.2), EMBase (1974-2012.2), Cochrane Library (Issue 1, 2012) and CBM (1978-2012.2) to find randomized controlled trials of plate versus kirschner wire in treatment of the distal radial fractures.Data analysis were performed with RevMan 5.0 software.Results Four RCTs were included.The results of Meta-analysis showed that: there were statistic differences between plate fixation and kirschner wire fixation about 3 months' follow-up DASH scores [ MD =-9.63, 95% CI (-15.49,-3.77), P = 0.001 ] and infection rate [ RR = 0.16, 95% CI (0.04, 0.60), P = 0.006 ], but there were no statistic differences about 1 year's follow-up DASH scores [ MD =-2.86, 95%CI (-9.44, 3.72), P=0.39], 3 months and 1 year's Mow-up grip strength [ MD = 7.28, 95% CI (-0.24, 14.79), P=0.06; MD=-7.57, 95%CI(-24.23, 9.09), P=0.37], the CRPS rate [ RR = 0.28, 95%Cl(0.06, 1.32), P=0.11 ] and the carpal tunnel syndrome rate [ RR =0.41, 95%Cl(0.06, 2.70), P=0.36].Conclusion Current evidence reveals that plate fixation is superior to kirschner wire in treatment of distal radial fractures in DASH scores and reducing infection rate, but there are no differences between them in grip strength and complications.关键词
