Abstract
Objective To analysis the clinical characteristics of pulmonary hypertension( PH) complicating pregnancy,to explore the monitoring as well as the treatment during perinatal period, the delivery time and the way. Methods Clinical data of 37 cases of pregnant women with pulmonary hypertension between Jan. 2005 and Apr. 2011 were analyzed and theywere divided into three groups; 19 cases of slight group [ pulmonary hypertension from 30 mmHg (1 mmHg = 0. 133 kPa) to 49 mmHg] , 10 cases of moderate group ( pulmonary hypertension from 50 mmHg to 79 mmHg) and 8 cases of severe group (pulmonary hypertension equal to or higher than 80 mmHg). To analysis the heart condition and the maternal and fetal outcomes. Results (1) In these cases, 18 cases were rheumatic heart disease, 11 cases were congenital heart disease,5 cases appeared with the perinatal heart disease, 1 case was hypertrophic cardiomyopathy, 1 case was idiopathic pulmonary arterial hypertension(IPAH) and 1 case heart structure was normal, in which 4 cases were rheumatic heart disease and 5 cases were congenital heart diseases. (2) The rate of NYHA class I-H was 68. 4% (13/19) in slight group. The rate of NYHA class Ⅱ ~ Ⅲ was 70% (7/10) in moderate group. The rate of NYHA class Ⅲ ~ Ⅳ was 75% (6/8) in severe group. (3)Caesarean section rate was 86. 5% (32 /37) among all patients,and 1 was vaginal delivery, 1 mother was death. 4 cases were induction of labour in second trimester,preterm labor 21. 9% (7 /32) ,3 cases were asphyxia newborn,4 cases were low birth weight infant, 1 infant was death. Conclusion Pulmonary hypertension (PH) complicating pregnancy is a high-risk pregnancy. The rate of heart failure increases gradually with the severity of pulmonary hypertension. The rate of maternal mortality and fetal loss increases in pregnant women with pulmonary hypertension complicating cardiac disease. The prognosis depends on whether the mother and perinatal infants with pregnancy complication. Cesarean section is more suitable for those in the third trimester.关键词
