胃肠道恶性肿瘤围术期肠内免疫营养对术后免疫功能的影响
Effect of perioperative enteral immunonutrition on postoperative immune function in patients with gastrointestinal cancer

Abstract
Objective To study the effect of nutritional enteral support on postoperative immune function and postoperative complications in patients with malignant gastrointestinal cancer (MGC). Methods Eighty - seven MGC patients were randomly divided into immune nutrition group (n = 29), general nutrition group ( n =29) and control group ( n = 29). The immune nutrition group orally took Ruineng 1 000 mL/d 5d preoperatively, and received Ruineng infusion in jejunum fistula d1 - d7 postoperatively. The general nutrition group orally took Rui Su 5 d preoperatively, and received Ruineng infusion in jejunum fistula d1 - d7 postoperatively. The control group received no perioperative nutritional support. The immune indicators, inflammatory markers, postoperative complications and bowel function recovery 5 d preoperatively and 8d postoperatively were detected. Results Compared with the preoperative, IgA, IgG and IgM levels were significantly higher in the immune nutritional group than in the general nutrition group (P<0.01 or P < 0.05). Compared with the general nutrition group and control group, the CD4, CD4 /CDg, NK lymphocyte count and lymphocyte ratio of the immune nutrition group increased significantly after surgery. The difference was statistically significant (P<0. 01). CRP were significantly higher in the three groups after surgery (P<0. 01). Compared with the general nutrition and the control group, immune and nutritional group had more increases in CRP (P<0. 01). Infectious complications and overall incidence of complications in the immune nutrition group were significantly lower than those of the general nutrition group and the control group ( P < 0. 05). The average hospital stay of the immune nutrition group was significantly shorter than those of the general nutrition and control groups (P<0.01). Conclusion Perioperative enteral nutrition support can improve the nutritional status of patients with gastrointestinal cancer with shorter hospital stay and less incidence of complications.关键词
