腹腔镜上消化道溃疡穿孔修补术58例临床疗效观察
Therapeutic observation of 58 patients with upper gastrointestinal ulcer perforation repair by laparoscope

Abstract
Objective To observe the clinical efficacy and safety of upper gastrointestinal ulcer perforation repair by laparoscope, and to summarize the experience of diagnosis and treatment. Methods Ninety-eight patients with upper gastrointestinal ulcer perforation in our hospital during the period of April 2009 to Feb 2012 were divided into laparoscopic group and laparotomy group according to the surgical approaches. The clinical efficacy and complications of the two groups were observed. Results The operation time in the laparoscopic group was longer than that in the laparotomy group, but the difference didn't show statistical significance (P >0. 05). The amount of blood loss in the laparoscopic group was obviously smaller than that in the laparotomy group, and the difference was statistically significant (P<0.01). The pain scoring, ambulation time, flatulence, number of patients using analgesia and the length of hospital stay in the laparoscopic group were smaller than those in the laparotomy group. The difference showed statistical significance (P<0. 01). There was one case of celiac residual infection in each group. Conclusion Upper gastrointestinal ulcer perforation repair by laparoscope has curative effect, high security, rapid postoperative recovery, and it is worth of being recommended in clinical applications.关键词
