腹腔镜阑尾切除术学习曲线的研究
The learning curve for the laparoscopic appendectomy

Abstract
[Objective] To evaluate the outcome of the laparoscopic appendectomy and define the learning curve for the laparoscopic appendectomy. [Methods] Consecutive patients with acute appendicitis who underwent (LA) from 2009 to 2010 were reviewed. The patients were divided into six groups (11 cases in each group) by operative sequence. Data on patient demographics, clinical and outcome variables including operative duration, conversion to open surgery, complications, and length of stay were analyzed. The learning curve for the laparoscopic appendectomy was established through the moving average and ANOVA methods. [ Results ] There was no significant difference a— mong the patients in the six different phases with respect to age, sex, weight and pathology. The patients were divided into group A who underwent the operation in the early period (before the learning curve) and group B who underwent the operation in the later period (after the learning curve). The average operative duration for group A was (76± 17.5) minutes, but it was (56±14.8) minutes for group D (P =0.000). There was three patients converted to open surgery in group A. group B was one case, there was a slightly higher in group A, but it was no statistical significance. Significant differences were not shown in the hospitalization period and infectivity complications. [Conclusion ] Operative duration and conversion can be reduced with increasing experience of LA, In this study, the learning curve for a laparoscopic appendectomy was about thirty cases.
