X线及内镜引导下放置鼻空肠营养管在危重患者中的应用效果比较

Abstract
Objective To investigate the efficacy and complications of the placement of nose-jejunum nutrition tube guided by X-ray super smooth guide wire and Endoscopic in critically ill patients. Methods 43 patients in need of enteral nutrition therapy in the Department of Intensive Care Medicine from July 2010 to January 2012 were randomly divided into group A (place nose-jejunum nutrition tube guided by super smooth guide wire) and group B (place nose-jejunum nutrition tube guided by gastroscopy). The placement successful rate, placement time, the incidence of placement related complications (nasopharyn-geal bleeding, upper gastrointestinal hemorrhage, aspiration, abdominal pain and abdominal distension, dyspnea) and the incidence of long-term complications (catheter translocation, pharyngitis, pulmonary infection) were compared between the two groups. Results The placement time, incidences of abdominal pain and abdominal distension and dyspnea between the two groups showed statistically significant differences (P < 0. 05). The successful rate of placement, incidence of nasopharyngeal bleeding, upper gastrointestinal hemorrhage and aspiration showed no statistically significant differences between the two groups (P>0. 05). The incidence of catheter translocation between the two groups showed statistically significant difference (P < 0. 05). The incidences of pharyngitis and pulmonary infection between the two groups showed no statistically significant differences (P >0. 05). Conclusion The placement of nose -jejunum nutrition tube guided by X-ray super smooth guide wire and Endoscopic in critically ill patients are both safe and effective method. Placement guided by X-ray super smooth guide wire can be regarded as the first choice, while for those who had duodenal stenosis and need mechanical ventilation and continuous veno-venous hemoftltration and who had poor peristole, placement guided by endoscopy is more appropriate.关键词
