低功率单极电刀凝切术与低温等离子刀和传统剥离术对成人扁桃体切除术的效果对比分析
Roles of Monopole Electrocoagulation,Coblation,and Conventional Tonsillectomy in the Surgical Removal of the Tonsils among Adult Patients

Abstract
Objective To compare the roles of monopole electrocoagulation (ME), coblation, and conventional ton-sillectomy (CT) in the surgical removal of the tonsils in adult patients. Methods Ninety adult patients who had undergone tonsil surgeries in our hospital from June 2010 to June 2011 were equally divided into three groups based on their surgical procedures: ME group, coblation group, and conventional tonsillectomy group. Parameters including operation duration, intra-operative blood loss, degree of pain 6 h, 24 h, 48 h, and 72 h after operation, and the average duration of pain were recorded. The depth of thermal injury was further evaluated under microscope. Results The operation duration and intra-operative blood loss were significantly shorter/less in ME group than in coblation group and CT group (P <0. 05). The degree of pain 6 h after surgery in ME group was higher than in coblation group (P < 0. 05), although no such significant difference was observed at other time points (P > 0. 05). The average duration of pain showed no significant difference among these three groups (P > 0. 05). The depth of thermal injury in ME group [ (0. 42 ±0. 16) mm ] was significantly shallower than in coblation group [ (1. 32 ± 0. 24) mm ] (P <0. 05). There was no significant difference of the duration of pseudomembrane between ME group and coblation group (P > 0. 05), although it was longer than in both of ME and coblation compared to CT group. Conclusion ME tonsillectomy can shorten the operation duration and reduce intra-operative blood loss without increasing the post-operative pain. Furthermore, ME causes a milder thermal tissue injury and is more affordable than coblation.关键词
