摘要

Abstract

Objective To explore the effects of localization of simultaneous EEG-functional MRI (fMRI) on epileptogenic focus in temporal lobe epilepsy. Methods Seventeen patients with symptomatic temporal lobe epilepsy -twere examined by simultaneous EEG-fMRI. The fMRI data collected during the time at epileptic discharge were analysed, and the area where the blood oxygen level-dependent signal changed by epileptic discharge was overlaid onto the image of MRI, then the localization of epileptogenic focus could be identified. Furthermore, the localization was compared with which described by intraoperative electrocorticography (ECoG) and deep electrodes. Results The activated regions in simultaneous EEG-fMRI were completely consistent with epileptogenic focus described by intraoperative ECoG and deep electrodes in 10 cases. Another 7 cases had a consistent central position with ECoG and deep electrodes, but the range was larger at simultaneous EEG-fMRI. Seizure of 14 patients was controled, and the seizure rate of the other 3 patients was obviously decreased after operation. Conclusion Simultaneous EEG-fMRI is accurate and noninvasive to localize the epileptogenic focus in temporal lobe epilepsy, which has reliabe guidance effect for the localization of operation.
