CT联合C臂机引导下椎体成形术治疗椎体骨折的疗效
Percutaneous vertebroplasty in the treatment of vertebral fracture guided by CT and C-arm fluoroscopy

Abstract
Objective To evaluate the effects and the application of percutaneous vertebro plasty (PVP) in treatment of compression fracture guided by CT and C-arm fluoroscopy. Methods 159 vertebral bodies in 131 patients of vertebral compression fracture were punctured under CT guidance, including 127 vertebral bodies through unilateral puncture and 32 vertebral bodies through bilateral puncture. Polymethylmethacrylate(PMMA) was injected by C-arm fluoroscopy guidance. Vasual analogic scale (VAS) and Oswestry disability index (ODI) were evaluated preoperatively and one week and six months postoperatively respectively. Results The puncture for all 159 vertebral bodies was successful at one time, the amount of PMMA injected into each vertebral body was from 3mL to 8 mL (mean 4.2 mL). Pain was decreased in all patients, VAS was decreased from 8.12 ± 0.57 before operation to 3.2 ± 0.32 at one weak postoperatively and 2.6 ± 0.47 at six months postoperatively (P ＜ 0.01). ODI was decreased from 63.14 ± 1.26 before operation to 33.25 ± 2.12 at one weak postoperatively and 22.31 ± 1. 57 at six months postoperatively (P ＜ 0.01) . Asymptomatic PMMA leakage was demonstrated by CT in 6 cases,two cases were complicated with adjacent vertebra fracture during follow-up. Conclusions PVP guided by CT and C-arm fluoroscopy is a safe and effective clinical therapeutic method. It provides accurate needle placement with CT guidance and real-time visualization of cement injection with C-arm X-ray machine guidance, then reducing the venture of PMMA leakage.
