纤支镜引导下经鼻气管插管在困难气道管理中的临床应用
THE CLINICAL OBSERVATION OF BRONCHOSCOPY GUIDED TRANSNASAL TRACHEAL INTUBATION IN DIFFICULT AIRWAY MANAGEMENT

Abstract
Objective: To investigate the clinical application value of the bronchofiberscope guided transnasal tracheal intubation in difficult airway management. Methods: To treat 125 patients who have difficult airway management in ICU and operation with fiberoptic bronchoscope guided intubation through nasal tracheal. The efficacy of intubation, tolerability, changes of vital signs and complications were observed and intubation management expertise was summarized. Results: A total of 124 cases of intubation succeeded, 95% of tracheal intubation was completed in 3 minutes. Intubation was well tolerated by 90% of the patients, although most patients have a transient increase of heart rate and blood pressure during intubation; 1 patient had trachea incision because of laryngeal edema. Twenty-three patients had airway bleeding during the intubation process, bleeding ceased after local oppression and drug hemostasis. Ten patients appeared sinusitis after intubation, recovered after administration with other artificial airway and antibiotic treatment. Conclusion: To administer difficult airway management with transnasal fiberoptic endotracheal intubation has high success rate, well tolerability and favorable safety.关键词
