巩膜外垫压联合玻璃体腔注气术治疗玻璃体切除术后视网膜脱离的临床分析
Scleral buckle combining with intravitreal gas injection for retinal detachment after vitrectomy

Abstract
Objective To observe the therapeutic effect of scleral buckling procedure combining with intravitreal gas injection on retinal detachment in eyes with vitrectomy and to investigate the indications and surgical techniques of the procedure. Methods From Jan, 2007 to Dec, 2011, the clinical data of 7 patients (7eyes), including 5 males(5eyes) and 2 females(2 eyes), with early retinal detachment treated by scleral buckling procedure combining with intravitreal gas injection in our department were retrospectively reviewed. All 7 eyes has fresh retinal redetachment with proper movements and without proliferative vitreoretinopathy(PVR) or development of proliferative membrane. Peripheral limited retinal redetachment not involving macular occurred in 4 eyes of the 7 eyes and retinal redetachment involving macular limited in the up half in other 3 eyes. During the procedure, the hole was directly located and frozen and the silicon sponge was sutured and pressed in the corresponding site. At about 4mm behind the limb us, inject vertically into vitreous cavity 0.3 ~ 0. 6ml ml pure C3F8 gas. Results All the patients' operations were completed well without any complications. Within the follow-up, eves achieved completely retinal reattachment (100% ). In 2 eyes without ideal formation of freezing plaque around the holes, the retinal holes were sealed after laser therapy in 5 to 10 days after operation. A small peripheral hole was found in another quadrant retinal and reattachment was not achieved in 1 eye postoperatively 37 days. After took this procedure a-gain, reattachment was achieved in this eye. Conclusion Choosing the proper case, intravitreal gas injection in scleral buckling surgery for early retinal detachment in eyes with vitrectomy is safe, simple, reliable, economical, convenient and effective.关键词
