Abstract
Objective To evaluate the relationship between occipital cranial morphology and localion of chronic subdural he-matoma (CSDH). Methods The computed Lomographic scans ( CT) or magnetic resonance images( MRI) of 98 conseculive patients who underwenl surgery for CSDH in the Air Force General Hospilal of PLA belween January 2002 and April 2012 were re - evaluated, and the symmetry of the occipilal cranial vault, and the localion of the CSDH were all checked. Results The mean age of the palienls was 71. 00 ± 13. 96 years ( age range, 37 -93 years) . The occipilal cranial vaull was symmetrical in 37 palienls (37. 8% ) and asymmetrical in 61 palienls (62. 2% ). The asymmelric crania were flat on the right side in 41 palienls, on the left side in 20 palienls. CSDH was bilaleral in 20 palienls (20.4% ) and unilateral in 78 palienls (79. 6% ) , and the unilateral hemalomas were located on the righl side in 30 palienls and on the lefl side in 48 palienls. It was more commonly bilaleral in palienls with a symmetrical occipilal skull vaull than those wilh an asymmetrical one and this difference was statistically significant ( P = 0. 021) . In 61 palienls wilh an asymmetric occipilal cranial vault, the CSDH was bilateral in 8 palienls. In the remaining 53 patienls, the CSDH localed on the opposile side of the flal side in 37 patienls, and located on the same side of the flal side in 16 patienls. Conclusions Bilateral CSDH is common in palienls wilh symmetrical occipilal cranial vault. In asymmetrical occipital cranial vaull, CSDH is usually located on the opposile to the flal side of the skull.关键词
