Analysis of the Unreasonable Prescriptions in Pharmacy Intravenous Admixture Services

Abstract
Objective To analyze of the unreasonable prescriptions in pharmacy intravenous admixture services and to ensure patients' medication safety. Methods Clinical pharmacists examined the prescriptions in PIVAS and analyzed the unreasonable prescriptions according to pharmacological knowledge and literatures. Results A totle of 2235 prescriptions was examined, and 72 pieces of unreasonable prescriptions were found,involving problems in incompatibility, improper choice of infusion carriers.unsuitable concentration,improper interval of drug administration and erroneous advising. With the intervention of clinical pharmacist, the rate of irrational prescriptions were reduceds from 6.7% to 1.63%. Conclusion PIVAS can decrease the irrational administration and guide clinical rational use of drug. 关键词
