区域新农合双向转诊利益分配模式的研究
Investigation on Interest Distribution Model foRBidirectional Referral in Regional New Rural Cooperative Medical Service

Abstract
Objective To design a new interest distribution model for all parties of bidirectional referral so as to promote the system construction of bidirectional referral gatekeeper in regional new rural cooperative medical service. Methods Literature method and statistic analysis based on the datas from regional health administrative management system were conducted. And thus an interest distribution model for all parties of bidirectional referral was developed, and the feasibility of the model was tested by using the real data from the regional hospitals in 2010. Results The real data test showed that the interest distribution model was generally beneficial. For each year it could save RMB 29 960 000 yuan in total medical coast and 8 710 000 yuan for new rural cooperation service fund. It could averagely reduce 1 056 yuan per capita each time for patients participating in the bidirectional referral, and the medical institutions of the three levels could gain 420 000 yuan as the surrender part of the benefits, the township hospitals could increase 11 500 000 yuan of income, and the responsibility doctor could increase more than 1 093 yuan in yearly salary. The consultant doctor could gain 62. 5-580 yuan for each referred patient. Conclusion The original impetus of bidirectional referral comes from doctors from two - leveled hospitals. The core of interest distribution model is breaking - even and surrendering profits among different medical institutions. The establishment of win - win - patterned interest distribution for all related parties can promote the system construction of bidirectional referral gatekeeper in regional new rural cooperative medical service.关键词
