低分割立体定向放疗联合自体CIK细胞免疫治疗中晚期肺癌的疗效观察

Abstract
Objective To assess the clinical efficacy of hypo - fractional stereotactic radiotherapy ( CyberKnife ) combined with CIK immnotherapy for patients with advanced lung cancer. Methods 48 patients with advanced lung cancer admitted to our hospital from November 2010 to August 2011 were divided into CyberKnife treatment group ( CK group, 37 cases ) and CyberKnife combined with CIK immunotherapy treatment group ( CK combined with CIK group, 11 cases ) . Four to six months after treatment, short - term clinical efficacy was evaluated by WHO Tumor Curative Standard and improvements of Kamofsky scores ( KPS ) . Results The median prescribed dose in CK group was ( 35. 4 ± 5. 4 ) Gy with isodose line of 66% to 8% ; while the median prescribed dose in CK/CIK group was ( 35. 6 ± 6. 3 ) Gy with isodose line of 69% to 81%. Of the patients in CK/CIK group, two received four CIK transfusions and nine patients received two. No opportunistic infections or capillary leak syndrome occurred during the CIK cell infusions, and acute side effects were mainly fever. The effective rate was 64. 9% ( CR 10, PR 14 ) in CK group and 81. 8% ( CR 5, PR 4 ) in CK/CIK group according to radiographic and clinical re - examinations and the difference between the two groups was statistically significant (x2 =0. 48, P >0. 05 ) . The KPS between CK group and CK/CIK group showed no statistically significant difference before treatment ( P > 0. 05 ), but showed statistically significant difference after treatment ( P <0. 05 ) . The median percentage of CD3 +/CD1656++ of CIK cells was ( 18. 80 ±4. 16 )% before transfusion, the survival ratios were over 99. 5% and 2 × 10 CIK cells were infused into patients. Conclusion CyberKnife combined with CIK immunotherapy can significantly improve patients' life quality and local control ratio of cancer, so it should be further studied.关键词
