Abstract

Objective To study the changes of the amplitude-integrated electroencephalography (aEEG) tracings within five days after birth in preterm infants with intracranial haemorrhage (ICH) and explore the value of aEEG in early diagnosis in preterm infants. Methods From October 2010 to July 2011,44 preterm infants with ICH at the Division of NICU, Children Hospital of Nanjing Medical University were studied by aEEG within five days after birth. The results of aEEG were categorized into three groups (normal, lightly abnormal and severely abnormal aEEG) , while ICH was clinically classified according to the Papile stander (Grade Ⅰ , Grade Ⅱ , Grade Ⅲ , Grade Ⅳ ). The correlations between the results of aEEG, severity of ICH were analyzed, respectively. The values of aEEG in early diagnosis of preterm infants with intracranial haemorrhage were analyzed. Results Among the 44 preterm infants with ICH, 14 infants had normal aEEG(all with Continuous normal voltage) , 17 had lightly abnormal aEEG (16 with Discontinuous normal voltage, 1 with Discontinuous normal voltage and seizure)and 13 had severely abnormal aEEG(7 with Continuous extremely low voltage,3 with Burst suppression, 2 with Flat and 1 with seizures) .respectively. Eleven infants had ICH of grade Ⅰ , three infants had ICH of grade Ⅱ , six infants had ICH of grade Ⅲ and three infants had ICH of grade Ⅳ. By CMH square analysis and Spearman rank correlation analysis, the results of aEEG classification were correlated with the severity of ICH. Abnormal aEEG could predict the severity of ICH in preterm infants with a sensitivity of 42.4% , specificity of 100%, positive predictive value of 100% and negative predictive value of 36.7%, respectively. Conclusion Monitoring with aEEG in preterm infants with ICH within five days after birth could predict the severity of ICH.
