Abstract
Objective To explore the effects of dexmedetomidine on postoperative cognitive function in elder patients undergoing abdominal 'surgery. Methods 100 patients scheduled for abdominal surgery were randomly divided into 2 groups: dexmedetomidine group (1 μg/kg loading and 0.2 ~ 0.7 μg/(kg·h) maintenance, n = 50, Group D) and control group (n = 50, Group C). Both groups received 0.04 mg/kg of midazolam, 4 μg/kg of fentanyl, 0.5 ~ 1.5 mg/kg of propofol and 0.15 mg/kg of cisatracurium. Cognitive function was evaluated before anesthesia, 5 and 24 hours after surgery by mini-mental state (MMS). Results There was no significant difference of the general status between the two groups (P > 0.05). The operative fentanyl, remifentayl and propofol requirements were significantly lower in Group D than those in Group C (P < 0.05). There were no significant difference of MMS score before anesthesia between the two groups (P > 0.05), and the MMS score at 5 and 24 hour after surgery in Group D were significantly lower than those in Group C (P < 0.05). There was no significant difference of TNF-α and IL-6 levels before anesthesia and at 24 hour after surgery between the two groups (P > 0.05), and the TNF-α and IL-6 levels at 5 hour after surgery in Group D were significantly lower than those in Group C (P < 0.05). Conclusions Dexmedetomidine can reduce the occurrence of postoperative cognitive dysfunction, which may be related to its anti-inflammatory effects and the reduction of the amount of anesthetics.关键词
