Difference between End-tidal CO2 Pressure Measured by Philips MP20 and Arterial CO2 Pressure

Abstract

Objective To observe difference between end-tidal CO2 pressure(PetCO2) and arterial CO2 pressure(PaCO2) in the patients who undergoing mechanical ventilation and general anesthesia. Methods There were 874 patients needing surgical operations were randomly selected, including 410 males ranging from 14 to 84 years of ages and 464 females from 12 to 93 years old. The operations involved were 342 ones for liver, gallbladder, pancreas or spleen, 249 ones for gastrointestinal tract, 229 ones for thyroid or breast, 23 ones for lung, mediastinum or esophagus, as well as 31 other ones. Rapid induction, endotracheal intubation, combined intravenous and inhalation anesthesia and mechanical ventilation were performed, and after 20 min, arterial blood gas analysis was executed with Stat Profile Analyzer from Nova Biomedical Corporation, and Philips MP20 was used to monitor the end-tidal CO2 pressure. Results The patients' PaCO2 was significantly higher than PetCO2(P<0.01), and the difference between them was (4.88±2.13) mmHg. But in some patients(6 cases), PaCO2 was lower than PetCO2. Conchusion PaCO2 is regularly higher than PetCO2 in the patients undergoing mechanical ventilation. In some patients such as the ones complicated with low cardiac output or ventilation/perfusion ratio disorder, PetCO2 can't more really reflect PaCO2, which is usually overestimated, and underestimated occasionally. PaCO2 should be detected to avoid acidosis in patients under general anesthesia.
