Abstract
Objective To evaluate the predictive values of Essen Stroke Risk Score (ESRS) and Stroke Prognostic Instrument-Ⅱ ( SPI- Ⅱ ) scale for short-term recurrent cardiac and cerebral ischemic events in patients after onset of minor stroke and transient ischemic attack (TIA) . Methods A total of 167 patients with minor stroke ( n = 111) or TIA ( n = 56 ) , who visited the Department of Neurology, the Third Hospital of Hebei Medical University from March 2009 to October 2011 were enrolled prospectively. The clinical data of the patients were documented, including age, hypertension, diabetes, hyperlipidemia, and smoking habit. The ESRS and SPI- Ⅱ scale scores were conducted (ESRS≥3 and SPI-Ⅱ ≥4 were the high-risk population of the recurrent ischemic events) . The recurrence of cardiac and cerebral ischemic e-vents was followed up within 90 days after onset. Results ①Of the 167 patients, 21 ( 12. 6% ) had cardiac and cerebral ischemic events (one was myocardial infarction and 20 were cerebral ischemic events) . The incidence of ischemic events in patients with minor stroke was 7.2% (8/111) , and the incidence in patients with TIA was 23. 2% ( 13/56) . The recurrence rate of ischemic events was highest (7.8%) within 7 days after minor stroke and TIA. ②Of the 21 patients with recurrent ischemic events, ESRS ≥3 and SPI-Ⅱ≥4 were both 17 patients (81% ) . The area under receiver operating characteristic (ROC) curve of ESRS for predicting ischemic events was 0. 677 (0. 557 to 0. 797) , and that of SPI- Ⅱ scale was 0. 553 (0. 413 to 0. 694) . Conclusion ESRS and SPI-Ⅱ scale as the preliminary screening assessment of stroke recurrence, they have predictive values for short-term recurrent risk of cardiac and cerebral ischemic events in patients with minor stroke and TIA,the predictive value of ESRS is slightly higher.关键词
