Association between Serum Macrophage Migration Inhibitory Factor,Tumor Necrosis Factor-α and Chronic Complications in Patients with Type 2 Diabetes Mellitus

Abstract

Objective To investigate the changes of serum macrophage migration inhibitory factor ( MIF ) and tumor necrosis factor - α( TNF - α ) in patients with type 2 diabetes mellitus ( T2DM ) and their roles in the chronic complications of carotid arteries artherosclerosis and diabetic nephropathy in T2DM. Methods 97 T2DM patients admitted to our hospital from A-pril 2009 to February 2010 were divided into three groups according to the IMT detected by ultrasonometry: group A, 25 cases, IMT≤0.09 mm; group B, 50 cases, 0.09 mm < IMT < 0. 13 mm; group C, 22 cases, IMT≥0. 13 mm or plaques formation. Another 30 healthy people underwent physical examination in the same period were involved as group D. The general information and biochemical tests were detected in the four groups, especially the levels of serum MIF, TNF - α and hs - CRP and their correlations with carotid artery IMT and urinary albumin/creatinine ( A/C ) . Results ( 1 ) The levels of serum MIF, TNF - α and hs - CRP between the four groups showed statistically significant differences ( P < 0. 05 ) . Compared with group D, the levels of serum MIF and hs - CRP in group B and group C were significantly higher, and the levels of TNF - α in group A and group B were significantly higher ( P < 0. 05 ) . The levels of serum MIF and hs - CRP in group C were significantly higher than group A and group B ( P < 0. 05 ). (2) Spearman correlation analysis showed that serum MIF was positively correlated with course of disease (r =0.309, P=0.002), carotid artery IMT ( r = 0. 496, P =0. 000 ) and hs - CRP ( r =0. 415, P = 0.000), and serum TNF - α was positively correlated with blood uric acid ( r = 0. 232, P = 0. 026 ), carotid artery IMT ( r = 0. 215 , P = 0. 037 ) and urinary albumin/creatinine ( r =0. 213 , P =0. 036 ) . Multiple linear regression analysis showed that serum MIF ( β= 0. 000, P = 0. 001 ) and hs - CRP ( (3 = 0. 003 , P = 0. 022 ) were correlated with carotid artery IMT. Logistic regression analysis showed that serum TNF - α was a risk factor for albuminuria [ OR = 1. 069, 95% CI ( 1. 005, 1. 137 ), P = 0. 035 ]. Conclusion The over - expression of serum MIF may be involved in the atherosclerosis of T2DM patients, and TNF - α can reflect the degree of renal damage.
