Psychosomatic symptoms and quality of life in breast cancer patients

Abstract
Objective: To explore the relation of somatic symptoms to anxiety and depression in breast cancer patients, the relation of psychosomatic symptoms to quality of life, and the differences of psychosomatic symptoms and quality of life among patients with different therapies and survival times. Methods: A cross-sectional descriptive design was used in 315 breast cancer patients in Beijing. They were assessed with the Patient Health Questionnaire (PHQ-15), Patient Health Questionnaire Depression Module (PHQ-9), Generalized Anxiety Disorder Scale (GAD-7) and Quality of Life Questionnaire for Cancer Patient (QLQ-C30). The validated data of 255 subjects were analyzed. Results: The severity of somatic symptoms was positively correlated with depression (r = 0.44, P < 0.01) and anxiety (r =0. 56, P <0. 01). In patients with severe somatic symptoms, the prevalence rate of depression was 42. 3%, while the rate of anxiety was 50% , and the depression and anxiety scores were both correlated with quality of life (β= -0. 22, -0. 30;Ps <0. 01). The somatic symptoms, depression and anxiety scores were higher in patients with chemotherapy than in patients without chemotherapy [ (9.0 ± 5.0) vs. (6. 0 ± 4.1), P < 0. 01; (8. 3 ±6.0) vs. (4. 0 ±3.5), P <0.01; (5. 4 +5.0) vs. (3. 1 ±3. 7), P <0. 01]. The anxiety score was lower in patients with survival time of > 5 years than in patients with survival time of ≤5 years [ (5. 8 ± 5. 0) vs. (4. 3 ±4. 5), P < 0.05], while no significant differences were found on somatic symptoms, depression and quality of life. Conclusion: It suggests that there are significant relationships between depression/ anxiety and somatic symptoms and quality of life. The patients with chemotherapy may suffer more psychosomatic symptoms and poorer quality of life, and patients with survival time > 5 years may experience less anxiety but similar somatic symptoms, depression and quality of life in comparison with ones with survival time ≤5 years.
