类风湿关节炎患者外周血单核细胞Toll样受体4的表达及其与血清白介素-18水平变化的相关性
Expression and Significance of Toll-like Receptor-4 in Peripheral Blood Monocytes of Patients with Rheumatoid Arthritis

Abstract
Objective To investigate the relation of expression of Toll like receptor-4 (TLR4) on the surface of peripheral CD14+ monocytes and serum IL-18 level in patients with rheumatoid arthritis ( RA) . Methods The expression rate and mean fluorescence intensity ( MFI) of TLR4 on the CD14 positive monocytes in peripheral blood from 34 cases of patients with RA, 7 patients with osteoarthritis ( OA) and 39 healthy controls were determined by flow cytometry, and serum level of IL-18 was measured by ELISA. According to disease activity score 28 ( DAS28 ) , 34 RA patients were divided into two groups, the highlyactive RA group and the moderate-lower active group. Results The expression rates of TLR4 on the CD14 positive monocytes were [ (24. 58 ± 10. 13) % ] and [(32.47 ±12.40)% ] respectively in highly active RA patients and moderate-lower active RA group, both were higher than that of control group [ ( 13. 14 ± 5. 22) % ] , and OA group [ (10. 23 ± 3. 40) % ] , but MFI of TLR4 in RA patients of highly active group (47. 34 ± 19. 85 ) and moderate-lower active RA group (42. 56 ± 17. 41 ) were lower than that of normal subjects (59.38 ±28.05) and OA group (67.90 ±31.40). There is difference among the four groups (P<0. 05). The serum level of IL-18 in highly active RA patients [ (236.71 ± 39. 42)pg/ml] was higher than that in moderate-lower active group [ (195.32 ±20. 69)pg/ml] and control group [ (185.49 ±42. 96) pg/ml ] , ( P < 0. 05 ) . The expression rate of TLR4 correlated negatively with DAS28 ( P < 0. 05 ) and positively with IL-18. Conclusions There is difference in the expression of TLR4 between RA group and health controls. TLR4 might be indirectly involved in the pathogenesis of RA.关键词
