联合毛果芸香碱滴眼液治疗眼外伤房角后退继发性青光眼
The role of pilocarpine eye drops in traditional ocular hypotensive therapies for traumatic angle recession glaucoma

Abstract
Objective To investigate the intraocular pressure (IOP) lowering effect of pilocaipine on traumatic angle recession glaucoma patients that already received anti-glaucoma and anti-inflammation treatments. Methods Histories of 12 traumatic angle recession glaucoma patients treated in Shandong Eye Hospital between 2007 ~ 2012 were reviewed. In all cases, UBM and gonioscopy examination demonstrated visible trauma with various degrees of angle recession. At the time of admission, the mean IOP was 48.50 ±3.10 mm Hg, and mean pupil diameter was 5.15 ±0.70 mm. Primary damage was aggressively treated and inflammation was controlled by a joint regimen of systemic anti-glaucoma medications and tropical maleate timolol, brinzolamide, and brimonidine tartrate eye drops for 3 - 5 days. This regimen decreased the IOP to an average of 35.00 ±3.55 mm Hg,which was still not optimal. Pilocarpine (0.5% ) eye drops at 4 times a day was then added to the regimen and detailed records of IOP outcomes were recorded. Results Twenty-four hours after pilocarpine was initiated,the mean IOP fell to 21.90 ± 3. 87 mm Hg, an additional 13.10 ± 1.10 mm Hg drop from the original regimen. The pupil diameter decreased to 2. 10 ± 0. 31mm. Four patients eventually achieved normal IOP; eight patients received trabeculectomy when IOP was successfully controlled at a safe level. Conclusion Pilocarpine can effectively reduce IOP in traumatic angle recession glaucoma. This IOP lowering effect is independent to existing anti-glaucoma and anti-inflammation treatments.关键词
